In the present work, a high-performance liquid chromatography-mass spectrometry (LC-MS/MS) enantioselective method was developed for the separation and quantification of dextro and levo-methorphan and their pharmacologically relevant metabolites, dextrorphan and levorphanol, in human blood samples. Enantiomeric separation of methorphan and metabolites was performed on a polysaccharide-based chiral Lux Cell 3 column using a methanol/water mobile phase. Application of this method to human postmortem blood samples has confirmed overdoses of dextromethorphan, levomethorphan, and, in rarer cases, both.
